Job summary

Color Health is seeking a dedicated Registered Nurse to join the Virtual Cancer Clinic care team. You will support patients with education, triage, and care coordination through phone and telehealth, collaborating with physicians, NPs, and care advocates.

The ideal candidate offers a BSN, an unrestricted RN license with NLC, and at least 7 years of nursing experience including 5 in primary care. Strong empathy, communication, and tech skills are essential.

Qualifications

• BSN required.
• Unrestricted RN license with NLC multi-state eligibility.
• 7 years nursing experience with 5 years in primary care.
• Ambulatory/outpatient care experience preferred.
• Excellent communication and telehealth skills.

Responsibilities

• Provide triage, education, and care coordination via phone and telehealth.
• Conduct clinical assessments and coordinate follow-up care.
• Perform telephone visits for symptom reporting and escalation when needed.
• Follow-up on abnormal screening results and guide next steps.
• Document encounters thoroughly and maintain records.
