Job summary

Tribe513 is seeking a Referral Specialist to coordinate care between external providers and patients, ensuring timely referrals and smooth transitions. You will review clinical data to support medical necessity reporting and help establish provider relationships for clear communication.

The role supports the Referrals team with shared tasks and may offer remote work options, with travel as needed. Previous medical office experience is preferred.

Qualifications

• High school diploma or equivalent.
• Proficient in computer skills and typing.
• Proficient in Microsoft Office and electronic medical record systems.
• At least 1 year of experience working in a medical office.
• Working knowledge of medical terminology and interpreting physician progress notes/medical records.
• Exceptional communication skills with internal and external customers.
• Home internet access and computer required for remote option.

Responsibilities

• Sends outgoing referrals and diagnostic imaging orders to specialist offices and receives incoming communication regarding those orders being processed.
• Ensures that patients are scheduled in a timely manner consistent with diagnosis and level of care needed and communicates appointment information with patient/family/caregiver.
• Follows up with specialist offices on pending referrals to gain consult notes/medical records and coordinate continuation of care with the provider as required.
• Promotes excellent relationships between our practice and outside specialty offices.
• Serves all patients, families, caregivers, external specialty offices, community, and internal team members with grace and humility.
• Promotes an authentic and transparent work environment.
• Other tasks as assigned.
