• Three years of experience in patient care, care management, transition/discharge planning, or related fields.
• Excellent interpersonal and communication skills.
• Ability to adapt quickly as needs arise.
• Knowledge of available health resources.

Responsabilidades

• Screen, monitor, and prioritize referrals and caseload.
• Promptly contact new patients by phone to introduce the program and schedule initial assessment.
• Conduct follow-up calls to reinforce education, self-management, and care planning actions.
• Collaborate with care providers across the continuum to meet patient needs.
• Maintain HIPAA compliance and ensure confidentiality of patient information.
• Lead interdisciplinary care conferences and huddles as needed.
• Document activities accurately in the medical record and maintain daily workflows.
