Job summary

Trillium Health Resources is seeking a Re-Entry Care Coordinator to support individuals transitioning from care settings who are justice-involved and preparing for release from Department of Corrections or county detention facilities by providing care coordination and transition planning services.

The role develops transition plans, aligns services across providers and community partners, and addresses barriers to support successful and stable transitions.

Qualifications

• Must meet Qualified Professional (QP) status per NC requirements with at least one year of experience with the forensic population.
• Fully licensed by the NC governing board regulating Human Services professions, OR an RN licensed in NC with MH/DD/SA experience.
• Bachelor's or Master's degree in relevant field with required MH/DD/SA experience.
• Must reside in NC within 75 miles of Trillium's North Central Region and have a valid driver's license.

Responsibilities

• Coordinate care transition activities for recipients on the High Priority Re-entry list to support discharges.
• Develop and implement transition plans based on individual needs and requirements.
• Collaborate with providers to align services during transitions.
• Monitor transitions and address delays or barriers to care.
• Facilitate service connections and ensure continuity after transitions.
