Job summary

Trillium Health Resources seeks a Support Transition and Reentry (STAR) Manager to lead a specialty care team serving incarcerated adults with complex behavioral health needs. You will supervise coordinators, clinicians and peer support specialists, ensuring high-quality, person-centered care and effective transitions from incarceration to the community.

The role requires leadership, care management expertise, and familiarity with criminal justice systems and reentry programs.

Qualifications

• Associate’s degree with three years of experience in behavioral/mental health, substance use and/or IDD populations OR Bachelor’s degree with one year of experience.
• Must have a valid driver’s license.
• Must reside within 75 miles of Trillium’s Southern Region.
• Must be able to travel within catchment as required.

Responsibilities

• Provide direct supervision and support to QP level coordinators, licensed clinicians and certified peer support specialists.
• Monitor team’s quality of work and take corrective action as needed.
• Oversee complex cases ensuring sound decision making and wholistic client care.
• Support staff to effectively plan and coordinate member transitions from incarceration to the community.
• Collect data, assign cases, and conduct quality audits.
• Collaborate with internal departments and external systems including prisons/jails, probation/parole, BH/SU providers, and other stakeholders.
