Job summary

CareStar, Inc. is hiring a Recovery Manager to oversee Specialized Recovery Support Services in Allen and surrounding counties, Ohio.

This full-time role combines remote work with required field visits to assess needs, build personalized care plans, and connect clients with Medicaid and community supports. Responsibilities include evaluating eligibility, coordinating services across systems, and advocating for participants while ensuring program compliance.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or a related field is required.
• Preferred: Licensed Social Worker (LSW) or Registered Nurse (RN).
• Minimum 3 years of experience in home and community-based services within the last 5 years.

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments (e.g., Adult Needs and Strengths Assessment).
• Develop and maintain individualized, person-centered care plans.
• Coordinate and authorize services across behavioral health, medical, and community-based systems.
• Assist individuals in accessing Medicaid and other essential services.
• Monitor service delivery and ensure compliance with program guidelines.
• Serve as the lead advocate and care coordinator for each participant.
• Adhere to the CareStar Rule in performance of job responsibilities.
• Understand and comply with CareStar Policies and Procedures.
• Maintain confidentiality as related to patient information; violations will be treated seriously.
• Follow the Acceptable Use Policy for CareStar information systems.
