Job summary

CareStar, Inc. is seeking a Recovery Manager to support Crawford and surrounding counties in Ohio. This role blends remote coordination with field-based assessments to connect individuals with Medicaid, behavioral health services, and community resources.

You will develop person-centered care plans, conduct eligibility assessments, and lead advocacy and care coordination while adhering to CareStar policies. A bachelor’s degree is required; LSW/RN is preferred.

Qualifications

• Bachelor’s degree in a related field is required.
• LSW/RN preferred.
• Minimum 3 years of experience in home and community-based services (within the last 5 years).
• Experience with Medicaid and behavioral health systems.

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments (e.g., Adult Needs and Strengths Assessment).
• Develop and maintain individualized, person-centered care plans.
• Coordinate and authorize services across behavioral health, medical, and community-based systems.
• Assist individuals in accessing Medicaid and other essential services.
• Monitor service delivery and ensure compliance with program guidelines.
• Serve as the lead advocate and care coordinator for each participant.
• Adhere to CareStar Policies and Procedures.
• Maintain confidentiality of patient information and follow CareStar information security policies.
