CareStar, Inc. is hiring a Recovery Manager for Warren, Hamilton, Clermont, Clinton, Butler, and Montgomery Counties, Ohio. The role combines remote work with field visits to assess needs and coordinate services across behavioral health, medical, and community systems.

You will lead care coordination, help individuals access Medicaid, and develop personalized plans while complying with program guidelines and CareStar policies. A meaningful opportunity in a mission-driven organization awaits.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or a related field.
• Preferred: Licensed Social Worker (LSW) or Registered Nurse (RN).
• Minimum 3 years of experience in home and community-based services (within last 5 years).
• Experience with individuals with chronic conditions or severe mental illness.
• Strong knowledge of Medicaid, behavioral health systems, and local resources.

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments (e.g., Adult Needs and Strengths Assessment).
• Develop and maintain individualized, person-centered care plans.
• Coordinate and authorize services across behavioral health, medical, and community-based systems.
• Assist individuals in accessing Medicaid and other essential services.
• Monitor service delivery and ensure compliance with program guidelines.
• Serve as the lead advocate and care coordinator for each participant.
