Job summary

CareStar, Inc. is seeking a Recovery Manager to coordinate recovery services for individuals across Summit, Stark, Columbiana, Mahoning, Portage, and Wayne Counties in Ohio.

The role combines assessment, care planning, and service authorization with a focus on helping clients access Medicaid and essential supports. The position offers remote flexibility with field visits, a strong emphasis on person-centered plans, and collaboration with multidisciplinary teams to improve lives in the community.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or a related field.
• Preferred: Licensed Social Worker (LSW) or Registered Nurse (RN).
• Minimum 3 years of experience in home and community-based services within the last 5 years.
• Experience with individuals with chronic conditions or SMI; strong Medicaid knowledge.

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments.
• Develop and maintain individualized, person-centered care plans.
• Coordinate and authorize services across behavioral health, medical, and community-based systems.
• Assist individuals in accessing Medicaid and other essential services.
• Monitor service delivery and ensure compliance with program guidelines.
• Lead advocacy and care coordination for each participant.
• Follow CareStar policies and confidentiality rules.
