Job summary

UnitedHealthcare is seeking a Transition Care Coordinator to apply clinical knowledge in coordinating member transitions from nursing facilities to community settings for medically complex individuals. You will work with the Care Coordination Team to ensure access to services, educate members, and develop care plans that are cost-effective and outcomes-driven.

Qualifications

• RN or LBSW licensure
• 2+ years clinical experience as RN or LBSW
• 1+ years MS Office experience
• Excellent organization, communication and time management
• Reside within commutable distance to Wilson, Smith, and Davidson Counties
• Valid US driver’s license with up to 50% travel

Responsibilities

• Coordinate transitions from nursing facility to community or between community-based settings
• Incorporate evidence-based clinical practices into care coordination
• Deliver member-centered self-management support and education
• Ensure appropriate utilization of health benefits and services
• Conduct telephonic and face-to-face assessments per policy/timeframes
• Serve as advocate to resolve barriers to care
• Coordinate referrals to community resources
• Develop member-centered care plans
• Collaborate with Care Coordination Team
• Monitor hospital encounters and safety during transitions
• Refer decisions beyond authority for review
• Participate in Quality Improvement processes
• Document all aspects of care coordination in Health Plan system
• Maintain timely documentation for regulatory compliance
• Coordinate services per Health Plan policy
• Perform other duties as assigned
