Job summary

UnitedHealth Group's UnitedHealthcare field team is seeking a Field Care Coordinator to travel to enrollees' homes and facilities in Danville, VA and surrounding areas. You will assess needs, develop person-centered care plans, and work with the interdisciplinary team to deliver cost-effective care.

You will advocate for members, support transitions from hospital stays, and promote self-management while operating from a home-based office in a field-based role.

Qualifications

• Bachelor's degree in Social Work or Human Services (or related field).
• 3+ years of care coordination, behavioral health or healthcare experience.
• 3+ years experience with medical needs, elderly, disabled or communication barriers.
• 1+ years of MS Office (Word, Excel, Outlook).
• Driver's license and reliable transportation; ability to travel within territory.

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing chronic conditions and wellness.
• Collaborate with care team, providers, and community resources to implement care plans.
• Provide education and coaching to support member self-management.
• Support discharge planning and coordinate care transitions after ER, SNF, or inpatient stays.
• Advocate for members and families to ensure needs are represented.
