Job summary

UnitedHealthcare is seeking a Field Care Coordinator to assess and plan for an identified group of enrollees, traveling to homes, facilities, and community sites in Charlottesville, VA.

You will coordinate with the Interdisciplinary Team, develop person-centered care plans, educate members, and advocate for their needs while navigating Medicaid/Medicare guidelines in a field-based role with potential remote work for commutable distance.

Qualifications

• Bachelor’s degree in Social Work or related field
• 3+ years of experience in care coordination or healthcare environments
• 3+ years of experience working with elderly or medically complex members
• 1+ years of experience with MS Office (Word/Excel/Outlook)
• Valid driver’s license and ability to travel within assigned territory

Responsibilities

• Conduct comprehensive needs assessments in member homes or facilities
• Develop and implement person-centered care plans
• Coordinate with internal care teams and community resources
• Educate and coach members for self-management
• Support care transitions and discharge planning
• Advocate for members and families as needed
