Job summary

UnitedHealthcare is seeking a Field Care Coordinator to assess and plan for a diverse enrollee group, traveling to homes, facilities, and day programs to develop care plans. You will work with an interdisciplinary team to coordinate cost-efficient, high-quality patient care and advocate for members and families.

This field-based role features a home-based office with travel throughout Northern Virginia and surrounding areas.

Qualifications

• Bachelor's degree in Social Work or Human Services (or related field).
• 3+ years of experience in care coordination, behavioral health, and/or healthcare.
• 3+ years experience working with members with medical needs, elderly, or disabled.
• 1+ years experience with MS Office (Word, Excel, Outlook).
• Willing and able to travel within the assigned territory with reliable transportation and valid US driver's license.

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment covering medical, behavioral, functional, cultural, and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, social determinants of health, and medication management.
• Partner with internal care team, providers, and community resources to execute the care plan.
• Provide education and coaching to support member self-management of care needs and lifestyle changes.
• Support proactive discharge planning and coordinate Care Transition after ER visits or SNF admissions.
• Advocate for members and families to ensure needs and choices are represented and supported.
