Job summary

PossAbility, LLC seeks a Qualified Intellectual Disability Professional (QIDP) to oversee Individual Service Plans (ISPs) for individuals with Diverse Disabilities and Abilities in Illinois. You will develop behavior support plans, generate monthly program reviews, and collaborate across disciplines to ensure compliant, person-centered services.

Responsibilities include coordinating ISP/BSP development, maintaining regulatory standards with IDHS, and supervising staff as needed.

Qualifications

• Bachelor's degree required in healthcare, human services, public health, or related field.
• QIDP certification and active listing on the Department’s QIDP database.
• Five+ years of experience as a direct care worker for individuals with IDD.
• Ability to support individuals with IDD and develop effective support strategies.
• Knowledge of home care regulations and best practices.
• Strong leadership and communication skills.
• Valid driver’s license and proof of insurance.
• Current First Aid and CPR certifications.

Responsibilities

• Develop, implement, and monitor Individual Service Plans (ISPs) to protect rights and meet needs.
• Assist in developing and overseeing Behavioral Support Plans (BSPs).
• Prepare written monthly program reviews with data, narratives, and community involvement.
• Collaborate with therapists, nurses, and social workers for comprehensive support.
• Ensure compliance with IDHS regulations and other agencies.
• Serve as primary case manager for individuals across programs and external services.
• Provide training and supervise staff on program implementation.
