Job summary

Aviary Health is seeking a Clinical Chart Abstractor - Pulmonary Rehabilitation to review patient EMRs and determine eligibility for our virtual program. This is a part-time, 1099 contractor role that is fully remote, offering 15-20 hours per week with flexible morning or afternoon ET slots.

Qualifications

• 3-5 years of clinical experience in pulmonary care, respiratory therapy, exercise physiology, cardiac or pulmonary rehabilitation, or chronic disease management.
• Experience reviewing electronic medical records and interpreting clinical documentation.
• Familiarity with pulmonary conditions, particularly COPD, emphysema, and other chronic respiratory diseases.
• Working knowledge of pulmonary function tests, spirometry, and relevant diagnostic terminology.
• Ability to interpret ICD-10 codes and apply established clinical eligibility criteria.
• At least 3 years of experience with EPIC or another electronic medical record system.
• At least 2 years of experience with medical terminology and clinical chart review.
• Proficient computer skills, including experience with Google Workspace, Zoom, and electronic medical record systems.
• Strong written communication skills, with the ability to summarize clinical findings and eligibility decisions clearly and accurately.
• Excellent organizational skills, attention to detail, and the ability to manage multiple tasks independently.

Responsibilities

• Review patient EMRs to determine eligibility for Aviary Health's virtual pulmonary rehabilitation program.
• Identify qualifying pulmonary diagnoses and interpret clinical documentation.
• Verify referral requirements and coordinate creation or completion of referral documentation.
• Document eligibility determinations and communicate outcomes to enrollment teams.
• Maintain orderly records of chart reviews in the EMR and other systems.
• Escalate cases outside guidelines for further clinical review when needed.
