Aviary Health is seeking a part-time Clinical Chart Abstractor - Pulmonary Rehabilitation to work remotely as a 1099 contractor. The role involves reviewing patient EMRs to determine eligibility for the virtual pulmonary rehabilitation program, coordinating referrals, and ensuring documentation meets program guidelines.

Ideal candidates have 3–5 years of pulmonary care experience, strong EMR skills (EPIC), and excellent written communication.

Qualifications

• 3-5 years of clinical experience in pulmonary care or related fields.
• Experience reviewing electronic medical records and interpreting clinical documentation.
• Familiarity with pulmonary conditions including COPD and other chronic respiratory diseases.
• Proficient with EPIC or another EMR system.
• Strong written communication skills and attention to detail.

Responsabilités

• Provide daily chart reviews to evaluate eligibility for the pulmonary rehabilitation program.
• Review diagnoses, medical history, PFT results, and documentation to confirm eligibility.
• Identify qualifying pulmonary conditions and apply program criteria.
• Verify ICD-10 codes and supporting documentation for eligibility.
• Coordinate creation or completion of referral documentation as needed.
• Ensure referral requirements and signatures are in place and documented.
• Identify missing or conflicting information and coordinate follow-up with the team.
• Communicate eligibility determinations and outstanding documentation to the enrollment team.
• Maintain organized records of chart reviews and eligibility decisions in EMR systems.
• Escalate cases outside eligibility guidelines to clinicians or leadership.
• Assist with special projects related to enrollment and chart abstraction.
