Job summary

Aviary is seeking a Clinical Chart Abstractor for Pulmonary Rehabilitation to review patient EMRs and determine eligibility for Aviary Health's virtual program. You will identify qualifying pulmonary diagnoses, interpret clinical documents, and ensure referral requirements are met to support timely enrollment.

Qualifications

• 3–5 years of clinical experience in pulmonary care, respiratory therapy, exercise physiology, cardiac or pulmonary rehabilitation, or chronic disease management.
• Experience reviewing electronic medical records and interpreting clinical documentation.
• Familiarity with pulmonary conditions, particularly COPD, emphysema, and other chronic respiratory diseases.

Responsibilities

• Provide daily chart review support to the enrollment team by evaluating patient medical records for pulmonary rehabilitation eligibility and ineligibility.
• Review pulmonary diagnoses, relevant medical history, PFT results, and other clinical documentation to determine eligibility.
• Identify qualifying pulmonary conditions and review ICD-10 codes to confirm eligibility.
• Coordinate the creation or completion of referral documentation when needed.
• Verify referral requirements and provider credentials in the chart review process.
• Maintain organized records of chart reviews and eligibility decisions in the EMR.
• Escalate cases outside eligibility guidelines for clinical review as needed.
