Job summary

OCHIN, Inc. is seeking a Transformation Consultant for Digital Health to coach health centers on optimizing EHR workflows and clinical operations. You will map workflows, analyze data, and apply quality improvement principles to drive better patient outcomes.

The role is fully remote across the US, with travel up to 25–50% as needed to support implementation across diverse settings. You will collaborate with clinicians, leadership, and HIT vendors to inform improvements and ensure successful

Qualifications

• Master’s degree in healthcare or related field preferred; 3+ years of experience in healthcare teams; 4–5 years a plus.
• Strong written and verbal communication and coaching ability across diverse environments.
• Familiarity with improvement methodologies (e.g., Model for Improvement) and using data to drive improvement; experience with clinical quality metrics is a plus.
• Experience with electronic health records and workflow visualization tools; Epic experience preferred (telehealth and remote patient monitoring).

Responsibilities

• Support health centers to utilize HIT solutions to meet transformation goals and apply quality improvement principles.
• Organize, analyze, and present EHR or performance data to leadership with actionable insights.
• Collaborate with clinicians and leaders to assess and redesign workflows and align with new technology capabilities.
• Support deployment of digital health tools, including remote patient monitoring and patient portals.
• Manage multiple projects related to clinical improvement and cross-departmental work.
