Job summary

Vaya Health is seeking a Post Transition Coordinator - QP to proactively coordinate services for individuals in the Transitions to Community Living program. This remote NC-based role emphasizes housing first, tenancy retention, and collaboration with care teams to ensure appropriate supports.

The role requires strong problem-solving, high diplomacy, and experience with Medicaid or social services workflows.

Qualifications

• Bachelor's degree in Human Services is required.
• If the Bachelor’s is in another field, 4+ years of relevant experience are required.
• Two years of post-bachelor’s experience with the population served is preferred.

Responsibilities

• Coordinate post-transition planning and community housing support.
• Lead Treatment Team meetings with providers and care teams.
• Maintain clear documentation in EHR and TCLD; ensure timely records.
• Assist with budgets, CLA eligibility, and housing assistance documentation.
