Job summary

Qualifications

• Master’s degree in a Human Services field with clinical licensure.
• Two years post-human services bachelor’s degree experience or four years full-time post non-human services experience.
• Licensed professional licensure (LCSW/LCMHC/LPA/LMFT or RN).
• Proficiency with Microsoft Office and handling confidential information.

Responsibilities

• Manage an active caseload of transition planning for members/recipients.
• Work with manager to create a yearly target number of successful transitions based on state benchmark.
• Ensure that the Pre-Quality of Life survey is completed prior to lease signing date.
• Educate tenancy support providers about roles and responsibilities of the Transition Coordinator.
• Adhere to policy boundaries and do not provide services outside assigned scope of work.
• Coordinate monthly updates for transitioned members/recipients and complete auditing tools on time.
• Collaborate with community providers to ensure needed services are provided.
• Provide staffing and clinical consultation to team members as needed.
