Qualifications

• Bachelor's degree required; two years of post-bachelor's experience with MH/SU/IDD population or four years if degree is not in Human Services.
• Proven ability to coordinate services and maintain documentation.
• Strong interpersonal and organizational skills with attention to confidentiality.

Responsibilities

• Coordinate post-transition services to help members remain housed.
• Collaborate with behavioral health providers and care management.
• Maintain documentation in EHR and TCLD; manage caseload and assessments.
• Coordinate with providers to identify housing options and support planning.
