Qualifications

• Bachelor’s degree in Human Services preferred or related field.

Responsibilities

• Provide proactive intervention and coordination of services for TCL participants.
• Lead ongoing Treatment Team meetings and coordinate with providers and care teams.
• Document all contacts and interventions in the member’s EHR and TCLD.
• Support housing stability goals and collaboration with housing options when needed.
