Job summary

Beth Israel Lahey Health is seeking a Population Health Navigator to support patients with hypertension, diabetes, and cancer screening needs. This temporary, primarily remote role collaborates with primary care and population health teams to educate patients, schedule visits and screenings, and address barriers to care.

The PHN will work with teams using patient registries and EHRs, staying within HIPAA guidelines and maintaining confidentiality.

Qualifications

Responsibilities

• Integrates into primary care practices and performance teams to engage patients with hypertension, diabetes, and cancer screening needs.

• Manages patient registries and supports pre-visit planning, reminders, and referrals.
• Outreach patients via phone, portal, or messaging to schedule appointments and screenings.
