Job summary

HealthSnap is seeking a Nurse Care Navigator to support NY-based patients enrolled in chronic care management and remote patient monitoring. The role involves educating patients on hypertension, diabetes and other conditions, and coordinating with a patient care team including physicians and health systems.

Hours are Monday–Friday, 9:00am–5:30pm ET. You will build ongoing relationships with patients to empower self-management, document care plans, and participate in quality improvement

Qualifications

• Current, valid New York LPN/LVN license in good standing.
• 3+ years in primary care, cardiology, internal medicine, home care or chronic care management/RPM.
• Proficient with EHR and care management software.

Responsibilities

• Complete phone consultations with patients in care management/RPM.
• Educate patients on chronic conditions and lifestyle strategies.
• Maintain accurate, up-to-date patient records per protocol.
• Address barriers to care and improve adherence.
• Communicate patient needs to providers per approved workflows.
• Participate in training and quality improvement initiatives.
• Respond to remotely transmitted patient data per partner workflows.
