Qualifications

Responsibilities

• Review patient charts to verify eligibility for refill under EMRC.
• Gather and review clinical information to support refill requests.
• Prepare and document refill authorization requests per state laws and policies.
• Ensure clinical criteria and documentation are complete before pharmacist review.
• Route refill requests to the pharmacist for final verification.
• Communicate with pharmacists and care team about missing information.
• Identify safety concerns and escalate to the pharmacist.
• Maintain accurate documentation in the patient record.
• Demonstrate strong computer proficiency and navigate EHR systems remotely.
• Collaborate with a large remote team and build professional relationships.
