Job summary

Mass General Brigham Community Physicians, Inc. is seeking a Pharmacy Technician – Medication Adherence to join our Population Health Management team in Somerville, MA.

The role focuses on identifying adherence gaps, patient outreach, and ensuring Medicare Advantage Star ratings improvements. You will collaborate with pharmacists and care teams to resolve access barriers, educate patients on regimens, and support cost-reduction strategies across lines of business.

Qualifications

• High School Diploma or equivalent.
• Pharmacy Technician license or Technician Certification (CPhT).
• 2+ years of experience, preferably in a retail pharmacy setting.
• Familiarity with medication refill processes and pharmacy workflows.
• Strong communication skills and comfort engaging patients by phone.

Responsibilities

• Identify patients at risk for medication non-adherence using PHM reports and EHR tools.
• Support Medicare Advantage Star measures and adherence gaps.
• Conduct patient outreach to address refills, access barriers, and affordability concerns.
• Educate patients on refill timing, dosing, and adherence importance.
• Support adherence interventions (refill sync, automatic refills, reminders).
• Coordinate with retail, specialty, and mail-order pharmacies to resolve issues.
• Document interactions and outcomes in EHR/PHM systems.
• Track adherence-related quality gaps tied to Medicare Star Ratings.
