Job summary

You will perform data entry, monitor member status, and educate providers and members while maintaining PHI confidentiality and collaborating with care teams. Strong organization, communication skills, and familiarity with electronic health records are essential

Qualifications

• High School diploma or equivalent required.
• Current Certified Pharmacy Technician or 1 year experience in health plan, medication adherence or quality department.

Responsibilities

• Supports outreach with members and providers.
• Educates Medicare members to improve patient compliance and quality metrics.
• Performs accurate data entry in outreach systems.
• Reviews prescription records to confirm medication adherence.
• Educates providers on quality metric guidelines to improve compliance.
• Maintains knowledge of CMS STAR measures and HIPAA confidentiality.
