Job summary

Mass General Brigham is seeking a Pharmacy Medication History Technician to obtain comprehensive home medication histories for newly admitted patients, support medication reconciliation, and improve patient safety.

Under the supervision of Transitions of Care Pharmacists, you will document in the EMR, coordinate with physicians and nursing staff, and participate in quality improvement initiatives, with a hybrid work arrangement at 75 Francis Street and a 40-hour work week.

Qualifications

• High School Diploma or equivalent is required.
• State license as a Pharmacy Technician is required.
• PTCB certification is required.
• 1–2 years of pharmacy technician experience.

Responsibilities

• Obtain comprehensive medication histories for newly admitted patients.
• Verify allergy information and document thoroughly in EMR.
• Communicate medication history findings to physicians and staff.
• Review medication lists to resolve discrepancies and ensure accurate orders.
• Precept pharmacy technicians and students.
• Adhere to HIPAA and safety requirements.
