Job summary

Mass General Brigham Community Physicians, Inc. is hiring a Pharmacy Technician to support medication adherence initiatives across Medicare Advantage, Medicaid, and commercial plans.

The role involves outreach to patients, coordination with multiple pharmacy sectors, and adherence tracking within EHR/PHM tools, under pharmacist supervision. We offer a generous sign-on bonus of $3,000 and a full-time schedule, primarily remote with occasional in-office work at Assembly Row, Somerville, MA.

Qualifications

• High School Diploma or equivalent required.
• Pharmacy Technician license or CPhT certification preferred.
• 2+ years experience, Retail pharmacy preferred; familiar with refill processes and workflows.
• Strong communication with patients by phone and basic computer proficiency with EHRs/pharmacy systems.

Responsibilities

• Identify patients at risk for medication non-adherence using PHM reports, payer files, registries, refill history, and EHR tools.
• Support Medicare Advantage Star adherence measures and address gaps across populations.
• Conduct patient outreach via phone, portal, or text to address late refills and accessibility concerns.
• Educate patients on refill timing, dosing, and adherence importance under pharmacist guidance.
• Coordinate with retail, specialty, and mail-order pharmacies to support adherence interventions.
