Qualifications

• Doctor of Pharmacy (PharmD) or a recognized license-eligible degree.
• Current, active pharmacist license in the relevant jurisdiction.
• Experience in community, clinical, hospital pharmacy or related setting preferred.
• Strong knowledge of medications, interactions, contraindications, and adverse effects.
• Excellent patient counseling and communication skills.
• Familiarity with electronic health records and pharmacy management systems.
• Understanding of patient confidentiality and healthcare privacy requirements.

Responsibilities

• Review and verify prescriptions for accuracy, appropriateness, dosage, and interactions.
• Educate patients on medication usage, side effects, and precautions.
• Support medication therapy management and identify potential problems.
• Assess records for allergies, contraindications, and duplications.
• Communicate with patients and healthcare professionals about medication concerns.
• Provide remote medication counseling and answer questions.
• Maintain accurate, confidential patient medication records.
• Support adherence initiatives and safe medication use.
