Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide telephonic and video-based medication counseling, MTM, and adherence support from a home office. The role emphasizes compassionate patient interaction, clinical monitoring, and coordination with care teams to optimize therapy outcomes.

You'll hold a PharmD or Bachelor of Pharmacy and an active US pharmacist license, with multi-state licensure preferred.

Qualifications

• PharmD or Bachelor of Pharmacy degree required.
• Active US pharmacist license with multi-state licensure preferred.
• Ambulatory care/MTM experience is preferred; hospital and community pharmacists encouraged to apply.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to organizational standards.
• Support population-health and disease-management programs.
