Qualifications
- Doctor of Pharmacy or equivalent license with active state license.
- Experience in community or clinical settings preferred.
- Strong knowledge of medications, interactions, and adverse effects.
- Excellent patient counseling and communication skills.
- Familiarity with privacy rules and ethics.
Responsibilities
- Review and verify prescriptions for accuracy and interactions.
- Provide patients with medication usage instructions and side effects.
- Support medication therapy management and reconciliation.
- Communicate with clinicians regarding medication concerns.
