Job summary

Qualifications

• High School Diploma or GED.
• North Carolina Peer Support Specialist Certification.
• Must be able to travel within catchment as required.
• Preference given to applicants with a history of criminal justice involvement or MH/SU recovery experience.

Responsibilities

• Provide transition support to STAR recipients exiting incarceration.
• Educate members and guardians on navigating behavioral, physical and social services, and community resources.
• Coordinate transition linkages including housing, employment and schooling.
• Collaborate with care teams to ensure connections to community resources and supports.
• Engage with local justice systems in custody and community settings as needed.
