Job summary

Trillium Health Resources seeks a Peer Support Specialist to join the STAR team, guiding justice-involved adults through pre-release and post-release transitions and linking them to care management and community resources.

You will educate members and guardians about accessing services, coordinate with local resources, and help overcome barriers to reentry, including housing, employment, and transportation. Remote-friendly with NC residency required.

Qualifications

• High School Diploma or GED is required.
• North Carolina Peer Support Specialist Certification is required.
• Must be legally authorized to work in the United States without sponsorship.

Responsibilities

• Connect recipients to treatment and support services.
• Meet with justice-involved members in correctional facilities and community settings.
• Educate recipients and guardians about navigating BH, IDD, and social services systems.
• Assist with transition planning and linkages to housing, employment, and community resources.
• Collaborate with care teams to address barriers to reentry, including insurance and transportation.
