Job summary

Trillium Health Resources is hiring a Peer Support Specialist to support recipients in the STAR program, focusing on pre- and post-release care coordination for justice-involved individuals.

You will educate members and guardians about navigating behavioral health, IDD services, and community resources, while coordinating with care teams to meet transition needs and barriers to reentry in North Carolina.

Qualifications

• High School Diploma or GED.
• One year lived experience with a behavioral health condition and/or substance use.
• Must have a valid driver’s license.
• Must reside in North Carolina and within 75 miles of Pitt County.
• Must be able to travel within catchment as required.

Responsibilities

• Ensure that recipients are well connected and provide a warm hand off to identify needed treatment and support.
• Meet with Justice Involved members in-person in Correctional Facilities and in-person in the community to facilitate care management activities.
• Engage members who meet criteria for Transitions to Community Living (TCL) to provide education about permanent supportive housing, supports, services, and other community resources.
• Assist with transition planning and ensure delivery in the most effective manner with a focus on optimal quality outcomes.
• Utilize person centered planning methods to gather information and address the specific needs of the individuals.
