Job summary

Trillium Health Resources is seeking a Peer Support Specialist to join the STAR team, guiding individuals exiting incarceration through pre-release and post-release care coordination. You will educate on navigating behavioral health, IDD, and community services while coordinating school, housing, and employment links.

The role emphasizes collaboration with local justice systems and community providers to reduce reentry barriers while supporting recovery and independence.

Qualifications

• High School Diploma or GED required.
• At least 1 year lived experience with a behavioral health condition and/or substance use.
• North Carolina Peer Support Specialist Certification required.
• Valid driver's license required.
• Must reside in North Carolina within 75 miles of Trillium's Southern Region.
• Ability to travel within catchment as required.
• Must be legally authorized to work in the United States without sponsorship.

Responsibilities

• Connect recipients to needed treatment and supports with a warm hand-off.
• Meet justice-involved members in correctional facilities and in the community to support care management.
• Educate members about transitions to community living and permanent housing options.
• Assist with transition planning to ensure timely, high-quality outcomes.
• Collaborate with care teams to address barriers such as insurance, transportation, and housing.
