Elevance Health seeks a Peer Recovery Specialist in Ohio, focused on care coordination and recovery support for Wellness program members. The role supports engagement, discharge planning education, and collaboration with case management teams to ensure smooth service transitions.

Minimum qualifications include an AA/AS and relevant health services experience; certification as a recovery specialist/peer coach is required.

Formación

• Requires AA/AS and minimum of 2 years in health services or behavioral health or peer support.
• Valid, active recovery specialist/coach or peer/family support certification required.
• BA/BS or MBA preferred for advanced roles.

Responsabilidades

• Identify opportunities to engage members and families in forming a recovery network.
• Develop and implement on-site discharge planning education at Recovery and Resiliency sites.
• Collaborate with Case Management Teams as a member advocate in discharge planning and service transitions.
• Act as a resource for staff on decision making and problem solving; initiate and maintain contact with members and providers.
