Job summary

SCAN Health Plan is seeking a Peer Advocate in a remote, part-time role to support members with health education and guidance. You will make outbound calls, build rapport, coach members, and help them navigate care needs.

The position requires being a current SCAN member, a High School Diploma or equivalent, and comfort with computer systems and MS Office. Bilingual skills are a plus.

Qualifications

• High School Diploma or Equivalent Experience.
• Current SCAN Membership, required.
• Proficient with computers and call center software.
• MS Office and internet browsing proficiency.
• Excellent communication, including personal SCAN experience.
• Bi-lingual in Spanish, Korean or Chinese a plus.

Responsibilities

• Make outbound calls to members based on identified health interventions or quality initiatives.
• Build rapport with members and communicate personal SCAN membership experience.
• Coach members to create and implement strategies that minimize health risks.
• Encourage and assist members to seek care when needed.
