Job summary

Community Health Solutions seeks an RN On-Site Care Coordinator to provide on-site care coordination and education to Medicaid members in the Medically Complex Children’s Waiver program. Qualifications

• Minimum three (3) years pediatric clinical nursing experience.
• Current RN licensure in SC and able to practice.
• Willingness to travel locally to patient homes.
• Spanish language skills preferred.

Responsibilities

• Perform initial medical eligibility assessment upon referral.
• Complete in-home care assessments and required documentation.
• Educate members and arrange necessary education services.
• Develop and present care plans for provider approval and DHHS submission.
• Coordinate with Care Advocate Team and respond to inquiries and issues.
• Attend advocacy meetings and address complaints or concerns as needed.
• Ensure PHI handling complies with HIPAA/HITECH, and remote work safeguards.
