Job summary

The On-site Care Coordinator is a Registered Nurse who provides care coordination, education and outreach services on-site to Medicaid members in the Medically Complex Children’s Waiver program. This role is remote with periodic home visits within South Carolina, requiring SC RN licensure and the ability to travel locally.

Responsibilities include eligibility assessments, in-home level of care evaluations, patient education, and the development of care plans.

Qualifications

• A minimum of three (3) years of recent clinical pediatric experience is required.
• A bachelor’s degree in a health-related field is strongly preferred.
• Current RN licensure in states where business is conducted.
• Bilingual (Spanish) is preferred.

Responsibilities

• Perform initial medical eligibility assessment and in-home level of care assessment.
• Complete annual home visits, quarterly in-person visits, and monthly calls when a visit is not required.
• Educate members and arrange education services as needed.
• Develop and present care plans to the primary care provider for approval and/or revision; submit care plan to DHHS for approval.
• Coordinate with the Care Advocate Team to ensure required services and documentation are addressed.
