Clarity Pediatrics in the United States is seeking a Nurse Care Coordinator Lead (RN) for a fully remote, full-time role. You will lead a multidisciplinary team to coordinate care for children with chronic conditions, manage appointments, authorizations, and serve as the main contact between families, providers, and pharmacies.

Responsibilities include staffing supervision, HIPAA compliance, care coordination, and education for families.

Qualifications

• Active California RN license in good standing required.
• Leadership experience directing a clinical team.
• Proficient with electronic health record (EHR) systems.
• Strong organizational and interpersonal communication.
• Ability to work effectively in a remote, virtual environment.
• Bilingual English/Spanish used in patient communication.

Responsibilities

• Lead and supervise nurses, medical assistants, and care team.
• Coordinate clinical appointments and referrals to ensure seamless care.
• Provide patient and family education and medication authorizations.
• Maintain HIPAA and state nursing board compliance and documentation.
• Track KPIs and participate in quality improvement initiatives.
• Collaborate with physicians, APPs, psychologists and therapists.
