Job summary

Clarity Pediatrics is seeking a Nurse Care Coordinator (RN) to join our fully remote care team. You will coordinate clinical appointments, manage referrals, and educate families to support chronic pediatric conditions, starting with ADHD.

Under clinical leadership, you will ensure seamless care delivery, maintain medication authorizations, and serve as the main contact between families, providers, and pharmacies. Experience with EHRs and remote collaboration is essential.

Qualifications

Active California RN license in good standing (2- or 4-year degree in nursing accepted).
Demonstrated proficiency with multiple technology platforms and electronic health record (EHR) systems.
Strong organizational, communication, and interpersonal skills.
Ability to manage competing priorities and work in a remote environment.
Comfort working with diverse pediatric populations.

Responsibilities

Coordinate, schedule, and reschedule clinical appointments across the care team.
Manage referrals to specialist and external clinical services.
Proactively follow up with families to reduce gaps in care and no-show rates.
Support care transitions and continuity across the interdisciplinary team.
Provide clinical updates to referring physicians and coordinate lab work.
