Actalent is seeking a Pediatric RN Case Manager to coordinate care for medically complex pediatric members within a managed care setting. The role emphasizes needs assessments, risk stratification, and personalized care plans across multidisciplinary teams.

You will work in a hybrid/remote model based in Florida, with onsite days and EPIC training, supporting transitions from hospital to home and improving care outcomes.

Qualifications

• Active Florida RN license required.
• Experience in pediatric case management within managed care/population health.
• Proven ability to assess needs and create individualized care plans.

Responsibilities

• Conduct comprehensive needs assessments for pediatric members with complex conditions.
• Stratify risk and prioritize outreach based on acuity.
• Develop and update individualized care plans per guidelines.
• Coordinate care across providers and hospitals for seamless transitions.
• Identify recently discharged members and arrange follow-up care.
• Facilitate safe hospital discharges and arrange post-discharge services.
• Conduct proactive outreach to high-risk pediatric members and families.
• Provide disease-specific education to promote self-management and adherence.
• Collaborate with utilization management to ensure services are necessary and cost-effective.
• Ensure documentation complies with Medicaid and PCMH standards.
