Job summary

Harboroaks is seeking a Patient Navigator (Admissions Coordinator) for a fully remote role in the United States. The position requires a high school diploma and 1+ year in healthcare admissions, preferably in mental health or substance abuse.

Pay is $17.00/hr with a 10pm–6:30am EST shift Tuesday–Saturday. You’ll support the Intake & Needs Assessment team at the Pennsylvania Access Center, guiding patients through admissions with empathy.

Qualifications

• High school Diploma or GED required.
• One or more years’ experience in healthcare admissions required. Preferably in the mental health or substance abuse field.

Responsibilities

• Review admissions against approved criteria with Admission Counselor.
• Schedule assessments of voluntary patients.
• Respond promptly to inquiry calls.
• Handle clerical duties such as data entry and answering phones.
• Gather required clinical and demographic data from patients and sources.
• Assist patients and families in seeking treatment.
• Register patient to the assigned program.
• Complete admission paperwork accurately and timely.
• Refer inquiries to other agencies when not appropriate for facility admission.
• Coordinate with referral sources and transportation for patients.
• Maintain a positive, empathetic, and professional attitude toward customers.
