Job description

The Remote Patient Monitoring - Home Telehealth Case Manager is responsible for the nursing process when providing or supervising direct patient care. The RN coordinates care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. They provide care and counseling to promote health and prevent illness and injury based on the nursing process.

Duties

The Remote Patient Monitoring (RPM) Home Telehealth (HT) Registered Nurse (RN) Case Manager demonstrates leadership, experience, and creative approaches to managing complex patient care. Care is provided in the most cost-effective manner. Responsibilities include, but are not limited to:

• Executing position responsibilities with leadership and creativity.
• Providing care in a cost-effective and least restrictive manner.

Qualifications

• Basic Requirements:
• English Language Proficiency: Proficient in written and spoken English as per 38 U.S.C. 7403(f).
• Graduate of an accredited school of professional nursing (ACEN or CCNE accreditation).
• Option for individuals in master's level bridge programs or foreign nursing graduates with valid registration.
• Current, full, active, and unrestricted RN license in a U.S. state, territory, or D.C. (GNT exception applies).
