The Remote Patient Monitoring - Home Telehealth Case Manager is responsible for the nursing process when providing or supervising direct patient care. The RN coordinates care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. They provide care and counseling to promote health and prevent illness and injury based on the nursing process.

Preferred Experience: 2-3 years of RN experience; case management experience preferred.
