This position is responsible for ensuring the continuity of care in both inpatient and outpatient settings, utilizing appropriate resources within established contracts and patients’ health plan benefits. It involves facilitating care continuity using basic nursing knowledge and skills to ensure resource utilization and quality outcomes. The role performs care management functions on-site or telephonically as needed.

This is a fully remote role where compact licensure is strongly preferred; CT licensure is mandatory for this role.

Responsibilities
  • Consistently exhibits behavior and communication skills that demonstrate our company's commitment to superior customer service, including quality, care, and concern with each and every internal and external customer.
  • Prioritizes patient care needs upon initial visit and addresses emerging issues.
  • Virtually meets with patients, families, and caregivers as needed to discuss care and treatment plans.
  • Identifies and assists with follow-up of high-risk patients in various care settings.
  • Consults with physicians and team members to ensure successful care plan implementation.
  • Coordinates treatment plans and triages interventions appropriate to team members' skills.
  • Uses protocols and pathways aligned with disease management programs to optimize outcomes.
  • Monitors and coaches patients to maximize self-management using motivational interviewing techniques.
  • Oversees discharge provisions, including follow-up appointments, home health, social services, transportation, etc.
  • Works with the care team, demonstrating accountability for patient management and outcomes.
  • Maintains effective communication with physicians, hospitalists, care facilities, patients, and families.
  • Assists members to maximize benefits according to health plans.
  • Participates actively in Care Management Coordination Committee (CMCC) meetings.
  • Documents patient information and care plans in electronic systems.
  • Coordinates care with the interdisciplinary team on assigned caseloads.
  • Adheres to departmental policies and procedures.

Qualifications

Required Qualifications:

  • Active RN license in CT; ability to qualify for additional state licenses.
  • 2+ years of nursing experience in hospital, acute care, or direct care settings.
  • Renal, CKD, or Dialysis Care experience as a main focus.
  • Proficiency in MS Office and ability to work in a Windows environment.
  • Access to dedicated home workspace with high-speed internet.
  • Ability to work the specified schedule.

Preferred Qualifications:

  • BSN
  • Certified Case Manager (CCM)
  • Diabetic educator experience
  • ICU, Cardiology, or Critical Care experience
  • Telephonic case management experience
  • Discharge planning experience
  • Knowledge of hypertension and/or diabetes
