The Remote Patient Monitoring - Home Telehealth Case Manager is responsible and accountable for the nursing process when providing and supervising direct patient care. The RN coordinates care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. They provide care and counseling to promote health, maintain wellness, and prevent illness and injury based on the nursing process.
Basic Requirements:

Preferred Experience: 2-3 years RN experience, case management experience preferred.
