Job description

Summary

The Remote Patient Monitoring - Home Telehealth Case Manager is responsible and accountable for the nursing process when providing and supervising direct patient care. The RN coordinates care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. They provide care and counseling to promote health, maintain wellness, and prevent illness and injury based on the nursing process.

Qualifications

Basic Requirements:

  • Proficiency in English (per 38 U.S.C. 7403(f))
  • Graduation from an accredited school of professional nursing (ACEN or CCNE)
  • OR current registration for foreign graduates with degree validation
  • Current, full, active, and unrestricted nursing license in a U.S. state or territory
  • Graduate Nurse Technician (GNT) exception details

Grade Determinations: Details for Nurse I, II, and III levels, including educational and experience requirements, are specified. Refer to the provided link for detailed assessment criteria.

Preferred Experience: 2-3 years RN experience, case management experience preferred.

Physical Requirements: Visual acuity, hearing, manual dexterity, and ability to perform prolonged walking, standing, stooping, sitting, and bending. Ability to demonstrate nursing procedures requiring technical skills.

Duties
  • Execute responsibilities demonstrating leadership and experience in managing complex patient care.
  • Provide care in a cost-effective, least restrictive manner.
  • Apply care and case management principles using health informatics, disease management, and HT technologies to improve veteran health outcomes.
  • Assess and review patient data, identify potential complications, and facilitate timely interventions.
  • Coordinate with community providers and interdisciplinary teams, including mental health, social work, pharmacy, and nutrition.
  • Support self-management through patient education, health coaching, and motivational counseling.
  • Communicate progress, trends, and concerns within the healthcare team.
  • Participate in fostering an evidence-based practice culture and professional development.
