Job description

Summary

The Remote Patient Monitoring - Home Telehealth Case Manager is responsible for and supervises direct patient care, applying the nursing process. The RN coordinates care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. They provide care and counseling to promote health and prevent illness and injury based on nursing principles.

Qualifications

Basic Requirements:

  • Proficiency in English language (per 38 U.S.C. 7403(f)).
  • Graduate of an accredited nursing school (ACEN or CCNE). Alternatively, candidates in a master's level bridge program or foreign graduates with valid registration.
  • Current, full, active, and unrestricted nursing license in a U.S. state, territory, or D.C.
  • Graduate Nurse Technician (GNT) candidates may be appointed temporarily while pursuing licensure.

Grade Determinations: Detailed criteria are available online, outlining education, experience, and practice requirements for each RN level (I-III).

Preferred Experience: 2-3 years RN experience, case management preferred.

Physical Requirements

Visual acuity, hearing, manual dexterity, clear speech, and ability to perform long periods of walking, standing, stooping, sitting, and bending. Technical nursing skills required.

Duties
  • Lead and innovate in managing complex patient care using health informatics, disease management, and telehealth technologies.
  • Assess patients comprehensively, triage data, and intervene for potential complications.
  • Coordinate with community providers and interdisciplinary teams to optimize care.
  • Support self-management, provide education, and motivate patients.
  • Communicate patient progress, identify trends, and participate in evidence-based practice activities.
  • Contribute to a safe, professional, and collaborative environment.
