Upperline Health is seeking a remote Patient Engagement Specialist to contact at-risk patients, explain services, answer questions, and schedule initial visits across multiple locations. You will build trust, educate on wrap-around care, and collaborate with the care team to support patient needs.

The role requires 3+ years in a healthcare, patient-facing position, exceptional communication, strong Excel/EHR skills, and a proactive, detail-oriented mindset.

Qualifications

• Minimum 3+ years in a healthcare, patient-facing role with a track record of success in direct-to-patient engagement, sales, and/or marketing.
• Bachelor’s degree in healthcare-related field strongly preferred.
• Exceptional communication with an uplifting, personable manner and outstanding phone etiquette.
• Excellent written communication with ability to provide notes and updates via computer systems.
• Strong technology skills and knowledge of Excel and EHR/EMR platforms.

Responsabilités

• Manage patient engagement outcomes for assigned patients and own patient conversion targets.
• Meet and sustain metrics such as Quality Scores, Acceptance Rates, and Outreach Volume.
• Foster relationships with patients and caregivers via phone with compassion and collaboration.
• Educate patients about the benefits of Upperline Health’s wrap-around services.
• Schedule medical appointments across multiple providers and locations and support referrals.
