Job summary

Accompany Health is seeking a Patient Engagement Coordinator to ensure our patients receive consistent, high-quality engagement. This high-volume outbound role focuses on scheduling Annual Visits and follow-up appointments, with 135+ calls per day, using our CRM and messaging tools to keep records accurate and up to date.

You will educate patients on the importance of ongoing care, collaborate with clinical teams, and deliver a warm, empathetic experience.

Qualifications

• 2+ years of related experience in a high-volume outbound call environment, preferably in healthcare
• Excellent patient communication skills with the ability to build trust with seniors and other patients
• Strong computer skills and knowledge of G-Suite (Docs/Sheets)
• Ability to manage multiple tasks in a fast-moving, remote-first environment

Responsibilities

• Conduct outbound calls/texts to schedule Annual Visits and follow-up appointments
• Maintain a daily outbound call target of 135+ calls using structured call queues in CRM tools
• Send SMS and message-based outreach to confirm appointments
• Navigate CRM systems and spreadsheets while on calls
• Educate patients on the importance of ongoing care and follow-ups with clinicians
• Provide warm, professional assistance and address patient questions
• Collaborate with clinical teams to ensure visit readiness and follow-ups
• Deliver an empathetic and encouraging patient experience on every call
• Monitor scheduling metrics and support engagement goals
• Act as liaison between patients and care team to address questions about services
